The Baton Rouge Clinic and Cigna said Monday they have launched a “collaborative accountable care initiative” designed to improve patients’ health and make care more affordable.
The clinic, on Perkins Road, will monitor and coordinate all aspects of medical care for about 5,000 Cigna health plan members.
Registered nurses at the clinic will serve as clinical care coordinators, helping patients with chronic conditions, such as diabetes, heart disease or obesity and other health challenges, navigate the health care system.
The RNs will help patients get follow-up care or screenings, identify potential complications related to medications and help patients prevent chronic conditions. Patients with chronic conditions are most likely to see immediate benefits of the program.
The clinic’s chief executive officer was out of the office Monday and unavailable for comment on details of the initiative.
